Assist-Life Stage, Transition

- Housing, employment, or education

Assist-Personal Activities

• Support with personal care tasks such as bathing, dressing, and grooming
• Assistance with meal planning, preparation, and clean-up
• Help with domestic tasks including laundry, cleaning, and ironing
• Support with medication reminders and understanding instructions
• Ongoing monitoring and adjustment of support to meet changing needs
Accommodation & Tenancy
Accommodation and Tenancy support assists participants to obtain and maintain safe, suitable, and sustainable housing. This service focuses on building the skills and confidence needed to manage tenancy responsibilities, resolve housing-related issues, and live as independently as possible within the community.
• Assistance with finding and applying for appropriate accommodation
• Support to understand and meet tenancy rights and responsibilities
• Help with communication and liaison with landlords, housing providers, and real estate agents
• Support in developing daily living and tenancy management skills
• Assistance to prevent or resolve tenancy issues and sustain long-term housing stability
